Mullvad is a Swedish VPN service launched in Gothenburg in March 2009 and still owned outright by its two founders, Fredrik Strömberg and Daniel Berntsson. It asks for no email address or personal details — an account is just a random 16-digit number — and charges one flat price with no tiers, trials or discounts. The client software is open source under the GPLv3.
